The focus is on creating a caring environment dedicated to healing and wellness. Nursing practices are based on evidence and current research. They also adhere to policies, procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Responsibilities include coordinating, ensuring, and providing direct patient care activities. This includes conducting nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is essential. Providing patient and family-centered education is also a key component. Care coordination and individualized treatment planning are implemented as applicable. The RN performs and documents comprehensive patient assessments. They consider the patient’s culture, age, and developmental level. Identifying patient risk factors and presenting complaints is crucial. Objective and subjective data are collected for assessments. The RN verifies initial admission assessments obtained by healthcare team members. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Actions and interventions are designed to be relevant to the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family. The RN fully implements patient care therapies and procedures within the scope of practice. They explain procedures, treatments, and processes to patients and families. The RN initiates actions to alleviate patient stress and discomfort. They recognize and communicate changes in patient conditions appropriately. Ongoing assessments and responses to treatments are documented according to policy. Tasks are delegated appropriately, with oversight of team members. The RN monitors the accuracy and completeness of delegated patient care. Identifying and addressing educational needs of patients related to prioritized problems is necessary. Facilitating the patient discharge process is part of the role. Input into patient assignments considers available staffing, unit activity, and patient acuity. Active participation in unit-based activities supports organizational values and priorities. The RN promotes a positive group dynamic and healing environment. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. Participation in quality improvement activities and core measures is expected. A holistic perspective regarding patient care is maintained.
